When you opt in, you may receive the following text messages from Optimum Brain Neurofeedback Center:
Appointment-related messages: appointment date and time confirmations, appointment reminders, rescheduling notifications, pre-visit preparation instructions, and general office announcements.
Promotional messages: office updates, special offers, wellness tips, and information about our services (sent only with your separate promotional consent).
How to Opt In

The form includes separate, unchecked-by-default consent checkboxes for (1) appointment-related SMS messages and (2) promotional SMS messages. You must actively check each box to consent. You may also opt in by completing a paper intake form at our office or by providing verbal consent.
Consent Not Required
Consent to receive text messages is not a condition of booking an appointment or receiving services from Optimum Brain Neurofeedback Center. You may choose to opt in to one, both, or neither category of messages.

Message frequency may vary based on your appointment schedule and, if applicable, our promotional messaging schedule.

How to Opt Out
You can cancel the SMS service at any time. Just text "STOP" to the short code or phone number from which you received messages. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Carrier Liability
Carriers are not liable for delayed or undelivered messages.
No Sharing of Mobile Information
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Section 3: Appointment Scheduling and Cancellations
Optimum Brain Neurofeedback Center offers neurofeedback services by appointment. By booking an appointment, you agree to arrive on time and provide at least 24 hours' notice if you need to cancel or reschedule. We reserve the right to charge a cancellation fee for missed appointments or late cancellations, as communicated at the time of booking.
Appointment availability is subject to change. We will make reasonable efforts to accommodate your scheduling preferences, but we cannot guarantee specific dates or times.

The neurofeedback services provided by Optimum Brain Neurofeedback Center are intended to support brain health and wellness.
